On behalf of HCAM & MCAL, we are honored to invite you to attend the 2012 Annual Convention & Expo.

In celebration of our profession’s commitment to Michigan’s residents, this year’s Convention theme is rightfully and proudly, CARE MADE IN MICHIGAN.

Purpose
The purpose of this event is to promote enhanced leadership, management and standards of center/facility operations through quality education.

Target Audience
This event is designed for long-term care and assisted living professionals, including center/facility owners, corporate-level management and directors, administrators, directors of nursing, nursing personnel and other members of facility-based interdisciplinary teams.

Overview
Education:

• Up to 16 contact hours in 8 different professional disciplines
• Honorable guest speaker Lt. Governor Brian Calley
• Opening keynote presentation by nationally recognized humorist Tim Gard
• 3 plenary sessions including an AHCA and a NCAL Federal Policy Update and an interactive forum on Managed Care
• Innovative and timely breakout sessions on urgent and emergent issues

Celebration & Networking:
• Vendor Appreciation Celebration featuring a performance by the Musical Comedians, a DJ, dance contest & more! (Sun., Sept. 9)
• Chair’s Happy Hour, Banquet and Afterglow Celebrations (Mon., Sept. 10)
• Awards & Recognition Luncheon (Tues., Sept. 11)

Annual Convention Expo:
• Featuring more than 130 different exhibitors (at time of print)

LODGING ACCOMODATIONS
Once you have registered for the 2012 HCAM/MCAL Annual Conventi on & Expo, please secure your hotel reservation directly with the Amway Grand Plaza Hotel (connected to DeVos Place Conventi on Center) located at 187 Monroe Ave. NW, Grand Rapids, MI 49503.  A block of rooms have been reserved for participants at the discounted group rate of $139 per night plus applicable taxes. Self parking is complimentary for guests staying at the Amway Grand. ENTREE SELECTION: CHAIRS BANQUET
New this year, participants (included in registration Packages A or G only) will have the opportunity to select one of three delicious, Chef prepared dinner entrees including opti on A) Roast Pork Tenderloin with Peach Chutney; B) Pan Seared Halibut with Mediterranean Ragout or C) Marinated Tofu Steak with Butternut Squash Sauce and Pickled Red Onions to enjoy at this year’s Chair’s Banquet. Please be sure to indicate your entree preference on your registration form. If no preference is indicated, entree option A will be selected for you. Once selected (or if no selection is made), changes to entrees cannot be made. A ticket for your selected entree will be provided upon check-in at the HCAM/MCAL registration.  An entree ticket will be required at dinner service.

CONFERENCE ATTIRE
Casual business attire is appropriate for the whole of this conference; however, formal attire is encouraged for the Chair’s Happy Hour and Banquet on the evening of Monday, Sept. 10. Please remember that conference room temperatures are difficult to regulate; please dress in layers for your comfort.

PRINTING SESSION MATERIALS
Since 2010, in an effort to be environmentally responsible, HCAM/MCAL is trying to reduce the amount of paper used at educational programs. Electronic program materials and handouts will be accessible online at the association website at www.HCAM.org, before, during and following the conclusion of this program for registered participants to access and print.

It is at the sole discretion of each presenter to provide program materials and handouts. Instructions on how to access and print program materials will be e-mailed via “Know Before You Go” to all registered participants (if e-mail address is provided on registration form) in the weeks leading up to the conference.

Registration
Please utilize the form on the back cover of this brochure to register for the 2012 HCAM/MCAL Annual Convention & Expo. Please mail or fax completed registration forms.  Registration is also available online.

Early Registration Deadline
The discounted early registration rate is available to current HCAM or MCAL members or associate partners only, whose registration is received by HCAM/MCAL by Friday, August 17, 2012.

Event Confirmation
After your registration form has been processed, an event confirmation will be faxed to your attention. Please be sure to include a current fax number with area code on the form. If you do not receive a confirmation within 14 business days of submitting your registration form to our office, please contact HCAM via e-mail at eventconfirmation@hcam.org.

Payment & Applicable Rate
All registration fees are due at the time of registration. We accept Visa, MasterCard, American Express or Discover. We will also accept a check or money order payable to HCAM. Incorrect rates will be adjusted to reflect the correct applicable rate and automatically charged accordingly. Should you have questions in regards to your applicable rate, please contact Meggen Galbreath, Finance Specialist, by phone at (517) 622-6188 prior to submission.

Cancellation & Refund Policy
All refund requests must be made in writing by e-mail to the attention of Denise Cook at DeniseCook@hcam.org. Telephone cancellations will NOT be accepted. Individual registration fees, less a 25% processing fee, will be refunded for cancellations received in writing prior to Aug. 17. Cancellations received after Aug. 17 and no-show registrants, will not receive a refund nor a credit to a future program.

Substitutions
Individuals registered to attend this conference, but unable to attend, may send an alternate in their place. Please contact HCAM via e-mail at eventconfirmation@hcam.org with the first and last name, title, facility/company and e-mail addresses of both you and your substitute to process this request.

Dietary Restrictions and/or Special Accommodations
Individuals with special dietary restrictions, or those who require special accommodations to fully participate in this conference, should contact HCAM in writing by e-mail to DeniseCook@hcam.org no later than Aug. 17, detailing their request or restriction. It is the responsibility of each participant to notify banquet staff of their dietary request at each meal on-site.

Consent to Use of Photographic Images
Registration and attendance at, or participating in, HCAM and MCAL events and/ or activities constitutes an agreement by the registrant to HCAM and MCAL’s use and distribution (both now and in the future) of the registrant or attendee’s image or voice in photographs, videotapes, electronic reproductions and audiotapes of such events and activities.
