Open Enrollment for Recurring COVID-19 Testing Supply Allocation (SNF/AL)
The Michigan Department of Health and Human Services (MDHHS) reminds long-term care facilities that open enrollment for recurring testing supply allocation ends Friday, March 10, 2023.
Interested facilities not currently enrolled to receive a recurring shipment of COVID-19 tests from MDHHS should fill out the LTC Monthly Antigen Shipment Enrollment Form by March 10 to initiate recurring shipments.
This monthly recurring allocation of tests will replace the current ordering process, where facilities had to request supplies using an online form each time items were needed.
MDHHS will provide quarterly opportunities for sites to adjust allocations or opt-out of the auto-shipment process. If you already submitted the Monthly Antigen Shipment Enrollment form AND were contacted by MDHHS regarding enrollment, there is no need to submit again.
Quarterly Enrollment Opportunity: Each quarter MDHHS will re-open this enrollment survey for those sites that either missed the initial sign-up deadline or are newly established. The quarterly enrollment opportunity will be open for the first full week of each new quarter; the next open enrollment period is June 5-9.
Facilities will receive additional information prior to the start of auto-shipment. Facilities may still use the online ordering form for a contingency supply as needed. Questions or concerns? Please email [email protected].
