CVS Health Foundation Grant Opportunity

The CVS Health Foundation is requesting grant proposals from multi-sector partnerships in 14 states including Michigan seeking to improve older adult health. CVS Health Foundation will award five multi-year, $1 million grants through its Focus on Healthy Aging (FOHA) program.
