MI-FRI Update (SNF)

The Federal Survey and Certification Division within the Department of Licensing and Regulatory Affairs and Bureau of Community and Health Systems, has made a change to the incident reporting process.

Effective July 1, 2022, the 2-hour reporting form on their website will be discontinued. All providers must use the MI-FRI application to report incidents as required by the Centers for Medicare and Medicaid Services (CMS).

In the event the MI-FRI application is unavailable, please follow the instructions below. This announcement will also be posted HERE.

ALERT: If the MI-FRI state application is not available for various reason (state maintenance, local Internet disruption, etc.), a provider should “screen shot” the MI-FRI screen or message to demonstrate that the state reporting system was unavailable and attach the message to the initial incident report once the system is back up. It is also advisable to keep a copy of the screen shot with the other reporting documents for a possible investigation by the state survey agency.  If the screen shot or message does not contain a date or time, the provider should document the date and time to demonstrate attempted compliance with the reporting requirement. The facility is still responsible for submitting the incident into the State of Michigan’s MI-FRI system via MILogin as soon as possible.
