Review of CON Annual Survey Data (SNF)
MDHHS CON staff have notified all nursing facilities regarding the data submitted on the CON Annual Survey for 2021. The email message below was sent out to the contact listed on the survey when it was completed. The survey data is used to determine the bed need in each planning area which is your county for all counties except Wayne County. Please take the time to review the data for accuracy it makes a difference in the formula for the beds available.
“This email is being sent to all Facility Administrators and Data Contacts listed in Sections A and N of the 2021 CON Annual Survey for Nursing Home and LTC Beds. Please review the attached reports for the data reported in the 2021 Annual Survey listed for your facility.
Please ONLY respond to this email if you find that you have reported incorrect data. If a correction is needed, please respond with your Facility Name, Facility No., and the change(s) you need to make (example below).
If you need to make a change to section N in the 2021 CON Annual Survey please respond by close of business Friday, July 15, 2022.
