Attention! Review Your QAAP Assessment by January 2

The Michigan Department Health and Human Services (MDHHS) loaded into each provider’s “Archived Documents” area via CHAMPS the provider’s annual Quality Assurance Assessment Program (QAAP) assessment notice applicable to the State Fiscal Year of October 1, 2023, through September 30, 2024. Please review this document by close of business on January 2, 2024, at 5 p.m. and notify the department of any differences between the provider’s original census reporting to the department (from the Fiscal Year 2020 Worksheet B) and the census data on the notice. If you have questions or census changes, please email MDHHS or send a written notice via File Transfer.
