Team of Excellence Award

 

Nominations for 2025 are now closed. 

The Team of Excellence Award recognizes and celebrates a long-term care team for their exemplary contribution and unwavering commitment to delivering quality care for residents. A winning team is one that works to build a culture of excellence. One team of employees from a skilled nursing facility and one team from an assisted living community will be selected to receive the award. Winners will be recognized during the Awards Luncheon at the HCAM Annual Convention in September.

Criteria for the Team of Excellence Award application: 

    1. Submit a completed application with all required documentation before May 12, 2025. (If any of the required documents are missing or incomplete, the application cannot be considered.)
    2. Application must include names and titles of everyone on the team.
    3. Application must include contact information for the team and for the person completing the nomination entry.
    4. Application must include a 300–600-word essay describing why the team should be recognized as a “team of excellence.”
      1. Use examples to describe how this team stands out in the care they provide to residents. Examples could include ways the team demonstrates outstanding compassion, innovation or clinical excellence providing resident-centered care.
      2. Use examples to describe how the team contributes to the positive image of the long-term care profession in the community. Examples could include ways the team demonstrates the nursing facility’s or assisted living community’s commitment to being a good neighbor and contributing to the overall welfare of the neighborhood/local area.
      3. Be sure to discuss the significant positive results the team achieved. For instance, did this team’s actions lead to improved safety, higher quality care, happier residents and families, community recognition, etc.?
    5. Applications must include at least one letter of recommendation for the team. The letter could come from family members, vendors, physicians, community officials or others who can speak to why the group should be recognized as a team of excellence.
    6. Applicants must be employed at an HCAM member facility or community in good standing.
    7. All documents must be typed and uploaded in Word or PDF format.
    8. We strongly encourage at least one member of the winning team to attend the HCAM Awards Luncheon in September in Grand Rapids.
