FRI Reporting (SNF)

When reporting Facility Reported Incidents (FRIs) to the Michigan Department of Licensing & Regulatory Affairs (LARA) providers should be utilizing the MI-FRI tool located within Michigan’s MILogin portal. Although there is a reporting form that may be used, it is intended for use only when the MI-FRI tool is unavailable due to scheduled downtime for maintenance.
