FY 2025 SNF VBP Program June 2024 Quarterly Reports Now Available (SNF)
The June 2024 Quarterly Confidential Feedback Reports for the FY 2025 Skilled Nursing Facility Value-Based Purchasing (SNF VBP) Program are now available to download in the Internet Quality Improvement and Evaluation System (iQIES). These reports contain data for the SNF 30-Day All-Cause Readmission Measure (SNFRM) for FY 2023, the FY 2025 SNF VBP Program performance period. These results will be used for the FY 2025 SNF VBP Program year scoring and incentive payment calculations that take effect October 1, 2024.
Members can submit requests for corrections to their FY 2023 readmission measure rate up until July 3, 2024. Corrections are limited to errors made by CMS readmission measure ratings. Submit corrections to CMS via email at [email protected] and include in the subject line “SNF VBP Review and Correction Inquiry.” Make sure to include your CMS certification number, facility name, correction and reason for requesting the correction.
To find your report in IQIES, follow these steps:
- Log into iQIES using your Health Care Quality Information Systems Access Roles and Profile (HARP) user ID and password. (If you do not have a HARP account, you may register for a HARP ID.)
- In the Reports menu, select “My Reports.”
- From the My Reports page, locate the MDS 3.0 Provider Preview Reports folder. Select “MDS 3.0 Provider Preview Reports” to open the folder.
- Locate the desired SNF VBP Program Quarterly Confidential Feedback Report.
- Select “More” next to your desired SNF VBP Program Quarterly Confidential Feedback Report, and the report will download through your browser. Open the file to view your facility’s report.
