LARA Requests Updated Information (SNF)

The Michigan Department of Licensing & Regulatory Affairs (LARA) Bureau of Community and Health Systems (BCHS) is requesting your help by completing a Health Facility and Agency Contact Information Survey to obtain contact information for your state licensed facility. The data will give up-to-date contact information for immediate licensing issues. In addition, the bureau will use the survey data to build a new state licensing management information system later this year. Please note that if you are completing this survey for more than one nursing home, you will need to use a new browser or a separate email account for each nursing home.
