Level of Care Determination Reminders for SNF

The Michigan Department of Health and Human Services (MDHHS) Provider Support reports that skilled nursing facility provider inquiries related to Level of Care Determinations (LOCDs) has increased. As a reminder for providers:

• The LOCD should be entered under the 10-digit Medicaid ID number for the beneficiary.
• If a Medicaid ID number has not been assigned for the resident all demographic information including the conducted-on date must be accurate.
• When submitting the LOCD, be mindful of the “verification notification” pop-up.
• Supporting documentation must be uploaded within 1 business day for the selected LOCD records. Review the LOCD Verification Review Tip for instructions on how to upload.
• Regularly monitor the LOCD end date.
• Conduct a new LOCD before the end date to ensure no gap between LOCDs. For instructions on how to enter a new LOCD, visit the Medicaid provider’s institutional alerts webpage.
