MDHHS Issues Guidance: SNF Transitions To Community (SNF)

On October 5, 2021 the Michigan Department of Health & Human Services (MDHHS) released L-Letter 21-63 directing community transition service agencies to reach out to nursing facility residents who may be interested in learning more about home and community based services. With this directive, MDHHS is asking nursing facility staff to accommodate this activity by identifying residents who have expressed an interest in returning to their home or community, or residents who may no longer meet the nursing facility level of care. Transition Navigators will make arrangements to meet with interested residents to identify needs and determine a transition plan if appropriate.
