MDHHS Recurring COVID-19 Testing Supply Allocation Change Opportunity (SNF/AL)

The Michigan Department of Health and Human Services (MDHHS) is offering long-term care facilities an open window to change their recurring COVID-19 testing supply allocations. From May 13 through May 17, 2024, members may adjust their testing allocations using this form. Please note that the form allows facilities to request supply increase or decrease or to opt out of recurring shipments. It is not to enroll in recurring shipments.
