MDS 3.0 Training (SNF)

The Department of Licensing and Regulatory Affairs (LARA) Bureau of Community and Health System will offer a virtual MDS 3.0 training. The focus will be section M-Skin Condition. Haideh Najafi, the State RAI Coordinator, will present the training.

This training will take place in three days (Thursday March 31, Wednesday April 6, and Thursday April 7). This training is free of charge, but there is limited number of participants for each day.

Pre-registration is required, and this training is for the MDS coordinator and that individual’s back-up. You may pre- register for only one day. The training and the material will be the same for all three days. You can pre-register only for one of the training dates.

The training is from 9:00 a.m. to 4:30 p.m. with one hour for lunch as well as two additional 10-minute breaks, one in the morning and one in the afternoon.

Pre-Registration process: There is no specific pre-registration form. Use the following format for pre-registration. Please e-mail Haideh Najafi at [email protected], before Friday March 25, 2022, with the following information:

1)   Names of the attendees including your disciplines and positions in your facility.

2)   Each individual’s e-mail address. Please be sure your e-mail is correct to ensure you receive the Zoom meeting invitation.

3)   Day-time Phone number, and/or cellphone number.

4)   Name and address of your facility.

5)   Date of the MDS training being requested.

6)   Please ensure all information above is included otherwise the registration may not be processed.

A Zoom meeting invitation will be sent to the email provided, which will include the information to access to the virtual training.
