Changes to section Q (Participation in Assessment and Goal Setting) include: 

• Reformatting of Question Q0100 (Participation in Assessment) to Q0110 (Participation in Assessment and Goal Setting) 
• Addition of a new question: Q0620, Reason Referral to Local Contact Agency (LCA) Not Made
