MDHHS recently revised its Statewide Transition Plan based on feedback from CMS. After clicking the link, scroll about half way down the page to reach the Statewide Transition Plan section.

Any comments regarding the revised Statewide Transition Plan, or requests for a written copy, may be submitted to [email protected]. MDHHS will be accepting comments until October 27, 2022. All comments on this topic should include a “Statewide Transition Plan Comment” reference in the subject line of the email.
