Updated Forms for HFAs

The Michigan Department of Licensing and Regulatory Affairs (LARA) recently shared a new form with HCAM. A Home for the Aged may use the “Quality Review Program Incident Review” form to supplement the facility’s incident report and document the quality review of an incident to demonstrate compliance with Administrative Rule 325.1901(k). In addition, LARA plans to share a second new form that will help HFAs complete the necessary annual review of the Quality Review Program. HCAM will continue to update you as these forms and changes are released.
