Updated Nursing Facility Per Diem List

On 11/15, the Michigan Department of Health and Human Services released the updated Medicaid nursing facility per diem list effective December 1, 2024.

This list, which includes applicable HCPCS codes, can be found here.

For Specific items included in the per diem prior to December 1, 2024, providers may refer to the Community Health Automated Medicaid Processing System (CHAMPS) Medicaid Code and Rate Reference Tool.
