Vaccine Passports Available (SNF/AL)

The Superior Health Quality Alliance team created a Vaccine Passport to help keep track of current and future vaccines. This poster is one version of the resource, but there is also a version that looks like a true passport that individuals could use to carry with them track vaccinations. To make these available to residents, complete this request form to have vaccine passports mailed directly to you.
