LARA & Headmaster Seeking Nurse Aide Testing Sites (SNF)

To ensure an efficient testing infrastructure in Michigan for Certified Nurse Aides (CNA’s), D&SDT-Headmaster is looking for long-term care facilities interested in becoming regional test sites for CNA candidates needing to take their manual skills competency exams. Your support in increasing availability of the number of test sites in Michigan would be greatly appreciated.

If you are interested in becoming a test site, please follow the 3 steps below:

1. Fill out the 1502MI – Test Site Agreement. Be sure to indicate one of the following about your test site:

• In-Facility/Regional – allows programs to reserve spots for their candidates in a test event scheduled at their site and allows candidates trained at other programs any open seats left in the event. This helps the testing infrastructure in the state of Michigan by opening your facility to those candidates trained elsewhere and keeps the test events full for the testing teams.
• In-Facility (Closed) – are for only your candidates. You would not allow any candidates trained elsewhere to fill up any empty seats after all of your candidates have been scheduled in the event.

1. Fill out the 1503MI – Test Site Equipment List – This form lists all of the equipment and supplies the test site is required to have available and in good working order on test days. 
2. Attach images of your test site layout, equipment, supplies, etc. when you submit the forms.
