Nomination for David Lebenbom Memorial Scholarship Award for Nursing Facilities (SNF Only)

Established in 2016 in memory of HCAM’s long-time legal counsel, the David Lebenbom Memorial Scholarship provides assistance to employees in meeting education expenses and advancing the long-term care profession. Serving as special legal counsel for the association in the 1960s, David Lebenbom helped pioneer the laws governing skilled nursing in Michigan for over 40 years. His firm, Lebenbom and Rothman P.C., continues to serve as HCAM general counsel to this day.

Scholarship applicants must be employed by an HCAM skilled nursing facility member in good standing for a minimum of one year and be enrolled in an education program related to long-term care at an accredited college or university. This can include, but is not limited to, administration, nursing, art, occupational therapy, and other related fields.

In 2024, HCAM will award up to eight $2,500 scholarships. Winners will be recognized at the HCAM Annual Convention, Sept. 16-18, 2024, in Grand Rapids.

Criteria for the David Lebenbom Memorial Scholarship Award application: 

  1. Submit a completed application with all applicable documentation by July 15, 2024. (If any of the required documents are missing or incomplete, the application will not be considered.)
  2. Application must include the nominee’s name and contact information.
  3. Application must include contact information for the person completing the nomination entry, if that person is different from the nominee. 
  4. Application must include a 300-600 word essay describing career objectives as they relate to the long-term care profession.
    1. Discuss how this scholarship will assist in achieving your short- and long-term goals.
    2. Describe any pertinent experiences you have had in the profession that have influenced you in choosing this career.
    3. Describe any positive impact you have had on residents, coworkers and/or the community in relation to your career in long-term care.
    4. Describe the reason you should be the recipient of this scholarship award.
  5. Application must include a letter of recommendation from current administrator regarding applicant’s job performance and length of time employed at the skilled nursing facility. Nursing facilities may decide whether or not they wish to require scholarship applicants to commit to working in a Michigan LTC facility for at least one year after completing the educational program for which the scholarship is awarded. This is not an HCAM requirement for receiving a scholarship. 
  6. All documents must be typed and uploaded in Word or PDF format.
  7. Applicant must be employed by a current HCAM member facility in good standing for a minimum of one year prior to applying.
  8. Applicant must be enrolled in an education program related to long-term care at an accredited college or university.
    1. Application must include academic transcript.
    2. Application must include documentation verifying applicant’s acceptance into educational program for which scholarship will be applied.

This application cannot be saved and continued at a later time. Please be sure you have all required items before you start.
