Certificate of Need Annual Survey is Now Open 

The Michigan Department of Health and Human Services CON Annual Survey is now open. All sections and payments are due by April 30, 2025. If you need help, visit the Annual Survey Training Videos website for section-by-section assistance. Videos can be paused and replayed as needed.
