DEADLINE: SNF QRP Data Due November 17 (SNF)

The submission deadline for the Skilled Nursing Facility Quality Reporting Program (SNF QRP) is fast approaching. All data must be submitted by 11:59 p.m. EST November 17, 2025. This includes:  

• Minimum Data Set (MDS) assessment data for April 1-June 30, 2025 (Q2 2025). 
• The Centers for Disease Control and Prevention (CDC) National Healthcare Safety Network (NHSN) COVID-19 Vaccination Coverage among Healthcare Personnel measure data for April 1-June 30, 2025 (Q2 2025). 

Providers should consider running applicable Internet Quality Improvement and Evaluation System (iQIES)/NHSN analysis reports prior to each deadline to ensure that all required data has been submitted. Recommended iQIES reports include the SNF QRP Provider Threshold Report and SNF QRP Review and Correct Report. The CMS iQIES Reports User Manual provides more information about how to run these reports in iQIES.

Also note: Swingtech Consulting (a Federal Miracle Tech partner) distributes notices on a quarterly basis to facilities that are not meeting Annual Payment Update data submission thresholds. If you need to add or change the email addresses to which these messages are sent, please email [email protected]. Include your facility name, CMS Certification Number and email update requests. At a minimum, it is recommended that a facility Administrator and Director of Nursing register for these emails.
