FY 2025 SNF VBP Program March 2024 Quarterly Reports are Now Available (SNF)

The March 2024 Quarterly Confidential Feedback Reports for the fiscal year (FY) 2025 Skilled Nursing Facility Value-Based Purchasing (SNF VBP) Program are now available to download via the Internet Quality Improvement and Evaluation System (iQIES). These reports contain interim stay-level data for the SNF 30-Day All-Cause Readmission Measure (SNFRM) for October 1, 2022-June 30, 2023 (Quarter 1-Quarter 3 of FY 2023, the FY 2025 SNF VBP Program performance period). CMS disseminates the SNF VBP Program’s quarterly confidential feedback reports via iQIES. To locate your new report in iQIES, please follow these instructions:

1. Log into iQIES using your Health Care Quality Information Systems (HCQIS) Access Roles and Profile (HARP) user ID and password. (If you do not have a HARP account, you may register for a HARP ID.)
2. In the Reports menu, select My Reports.
3. From the My Reports page, locate the MDS 3.0 Provider Preview Reports folder. Select the MDS 3.0 Provider Preview Reports link to open the folder.
4. Here you can see the list of reports available for download. Locate the desired SNF VBP Program Quarterly Confidential Feedback Report.
5. Select “More” next to your desired SNF VBP Program Quarterly Confidential Feedback Report and the report will download through your browser. Once downloaded, open the file to view your facility’s report.
