Five-Star Changes in Michigan (SNF)

On June 18, 2025, the Centers for Medicare and Medicaid Services (CMS) announced two upcoming changes to Five-Star, dropping the third most recent standard survey from the Survey Rating (July 30) and updating the risk adjustment methodology for four claims-based measures: Short and long stay hospital admissions and emergency department visits. The American Health Care Association (AHCA) analyzed the data and found that, nationally, 75% of all SNF providers will see no changes to their Survey Rating.

In Michigan:

  • 13.8% will lose a star
  • 75.5% will see no change in star rating
  • 10.7% will gain a star

Please note that some of these changes are due to regular events such as new survey results, PBJ data or quality measures, and not necessarily the CMS change to its Five-Star process.
